If you are uninsured, or if we do not participate with your insurance, one of our financial counselors will be happy to work out a financial plan prior to your visit.

To find out if we are in network with your carrier please call the number on the back of your insurance card.
Insurance co-payment is due at the time of service, if you are coming in for testing only, please check with your insurance company to see if a co-pay will be charged. If you do not bring your co-pay, your appointment may be rescheduled.
Please remember that your insurance is a contract between you, your employer, and your insurance company. You are personally responsible for any bill, or portion thereof, not paid by your insurance company.
